PremierCare Select Rx (HMO SNP) covers insulin and other diabetes drugs
with no deductible and no co-pay, including both Part B and Part D
drugs.*

All other Part D prescription drugs are covered under a low $100 annual
deductible and co-pays as little as $7 for a 30-day supply of generic

LOW HOSPITAL
COSTS
If you’re hospitalized, Select Rx (HMO SNP) members pay $100 per day
for the first five days. Compare that to Original Medicare with a
$1,100 hospital deductible and $275 per day co-pay for days 61 through
90.*
VISION, HEARING AND
DENTAL COVERAGE
With Select Rx (HMO SNP), you also get coverage for vision, hearing and
dental. Benefits include:
- $0
co-pay for one routine eye exam each year. Original Medicare
doesn’t cover routine eye exams.*
- $100
credit for glasses or $65 credit for contact lenses every year.
This routine vision coverage is in addition to Original Medicare
coverage for prescription eyewear after cataract surgery.*
- $500
credit for routine hearing exams and hearing aids every three
years. Original Medicare does not cover hearing aids.*
- Affordable
dental coverage, with co-pays as low as $12 for a routine dental
exam with fluoride treatment.*
