    NOTE: If you mail us your enrollment form, please provide (if applicable) a copy of your:

    • Medicare card
    • LIS letter
    • OMAP ID card
    • Notice of Credible Coverage

    Once we have received your enrollment form:

    • FamilyCare will verify your enrollment.
    • FamilyCare will call you and let you know you've been accepted.
    • FamilyCare will send you a Confirmation of Enrollment Letter and a New Member Packet.
